MEMBERS AND VOLUNTEERS - WHAT DATA DO WE COLLECT AND WHY?
Devizes and district phab hold contact details for all members, committee members and volunteers such as, name, dob, address, phone no’s, email addresses (if applicable), health conditions/disabilities, medication lists and next of kin names, addresses and contact numbers.
To comply with the new guidelines, we also need to hold your consent in holding the above contact details, your consent to the club to use photo’s of yourself in any phab publications, brochures, devizes and district phab website, phab magazine publications (wilts & swindon area committee phab magazine).
All data and welfare forms (paper versions) are held by the welfare officer (sarah gaiger) and is help in a locked box inside a locked cabinet and all computer held information will be securely stored and password protected.
The data we hold is mainly used to contact members in emergencies, if we need to cancel any club nights or outings, contacting members next of kin if needed, members welfare forms are also released when those members attend any phab holidays and outings.
The forms are placed individually in envelopes which are sealed, signed and dated by the welfare officer.
We do this as a precaution and hopefully they are not needed to be opened but in case of any accidents, incidents or other problems then the information contained may be needed by healthcare, emergency and the police depending on the circumstances.
All data held will only be used by those individuals who are require that information such as transport issues, contact issues, welfare issues and dbs paperwork.
The welfare forms and gdpr will be updated yearly at the same time as membership renewal from the 1st January.
All previous forms, deceased members forms and lapsed membership forms (which are currently held over for 1 year) will be securely destroyed by a cross shredder by the welfare officer.
Every individual has the right to ask to see all the data we hold for them, if you would like to see your data information then please contact the welfare officer by phone call, email or post to: -
